St Luke’s Hospice is a registered charity offering a range of specialist palliative care services to people with any life threatening, life limiting disease, predominantly but not exclusively cancer, such as Heart Failure, Multiple Sclerosis, Chronic Obstructive Pulmonary Disease or Motor Neurone Disease for example. Care and support is offered through a wide range of services that are tailor made to meet the needs of individuals. This care is extended to carers, families and friends.
Services are for age 16 years upwards except where stated otherwise, (ie: Lukes Counselling Service for Children & Young People)
All Hospice staff work closely with a wide range of health and social care professionals to achieve the highest level of care for patients and their families.
Operating 24 hours a day, 7 days a week OneResponse is a co-ordinating service offering support and advice for people with palliative care needs, or needing care at end of life. Our Community Palliative Care Team (CPCT) offers specialist palliative care, assessment, advice and symptom management for those in our community with life limiting illness and complex needs. On-going support is offered whenever there are complex issues throughout the patient journey, working as a specialist resource to health and social care providers across the locality. Specialist assessment for people affected by life limiting illness who need occupational therapy support is also provided, as well as support and education to all staff working within the community setting. Our Advanced Care Planning Team act as an expert resource to the health and social care workforce, by offering advice and information regarding care choices at the end of life and facilitating the advanced care planning process, incorporating the use of the nationally recommended End of Life Tools. Referrals can be accepted for anyone living in the Basildon and Thurrock districts, and also for the South Essex Lymphoedema Service. As the service provider we are pleased to be able to extend our established Lymphoedema clinical care, expertise and support throughout South Essex including , Thurrock, Basildon, Billericay, Wickford, Southend, Canvey, Castlepoint and Rochford
Referrals can be accepted to Hospice services from any health or social care professionals and also from patients and families where appropriate.
Referrals are regularly discussed and are considered on the basis of need.

Education and Professional Development Services
We have over 20 years of experience in caring for patient and families, our Education Team facilitate evidence based Palliative care training to the multi professional team working in palliative care or generic settings. We provide learning opportunities in palliative care that will enhance and improve end of life care for patients across the community.
