Operating 24 hours a day, 7 days a week OneResponse is a co-ordinating service offering support and advice for people with palliative care needs, or needing care at end of life. It is a central point of communication for patients, their carers’ and family as well as professionals.
OneResponse is for people aged 16 and over who live in South West Essex, this includes Basildon, Wickford, Billericay, Thurrock and (from April 2015) Brentwood.
The service is staffed by qualified professionals, including Clinical Nurse Specialists, registered palliative care nurses, end of life care facilitators and trained call assistants . Calls are responded to with empathy and friendliness offering appropriate advice and support.
Any patients, carers, families and professionals can access OneResponse under the following three categories appropriate to their needs following a telephone assessment, as follows :-
- Rapid Response :- Visit from an appropriate professional person (eg; qualified nurse/doctor/support professional) within 2 hours, for people who are identified as being in the last twelve months of life and assessed as in a crisis.
- Urgent Response:- Visit from an appropriate professional person within 24 hours
- Routine Response:- Visit from an appropriate professional or Key worker within 5 days
The Co-ordinated Care Register
The Co-ordinated Care Register is a key tool in providing co-ordinated care for supporting patient care and choices. It can be viewed by many agencies, including out of hours GP’s. OneResponse is based at and managed by St. Luke’s Hospice, in partnership with North East London NHS Foundation Trust (NELFT), working alongside Basildon & Brentwood and Thurrock Clinical Commissioning Groups, GPs, community nurses, agencies and specialists as well as other Hospice services to make sure high quality co-ordinated care is received .
